Instructions for Completing the 2012 Data Report for Projects with No Participants

Below is a guide for completing the data report for grantees who did not have any participants as of September 30, 2012. If you have any questions, please contact the AFI Resource Center Data Team (1-866-778-6037 or info@idaresources.org).   

Grantee Information Update Form

  • Fields will be pre-populated with information by OCS. Please review for accuracy. If any of the pre-populated information is incorrect, please contact the AFI Resource Center Data Team.
  • Add other contact information based on the type of contact (see sub-bullets for explanation). 
    • Main Contact: The person(s) who AFI staff should call first if there are questions concerning the grant or overall AFI project.
    • Alternate Contact: The person(s) who AFI staff should call if the main contact is unavailable.
    • Data Contact: The person(s) who compiles answers for this report.

Section A: Grant Information

This section asks for basic information about the AFI organization that is providing data about its AFI Project.

 For this section:

  • Questions A.1 - A.9 are pre-populated by OCS. Review and contact the AFI Resource Center Data Team if there are any corrections.
  • Complete: A.10 – A.17

Section B: AFI Project Structure

This section asks about the grantee organization, any partner or sub-grantee organizations, and the structure of the AFI project. Some of these issues may still be in the design stage, but try to answer them as to what was written in the grant application or your best guess at this time.

For this section:

  • Answer all questions.
  • For B.1:
    • If you know the structure of your AFI project at this time, select either “Single Agency” or “Network” from the drop-down menu.
      • If “Single Agencyis selected, then answer questions B.8 through B.15 accordingly.
      • If “Network” is selected, then answer questions B.16 through B.20 accordingly.
    • If you DO NOT know the structure of your AFI project at this time, select “Other” from the drop-down menu, and provide an explanation in B.1.a.
  • Complete: B.2 – B.4
  • Questions B.5 – B.7 ask about project staffing. If no time has been spent on this project, enter zeros.
  •  Adding Subgrantees/ Subcontractors. Click on the “Add New Subgrantee” button. If the project is a “Network Project” or “Other Project” but the subgrantees or partner organizations are unknown, create a subgrantee and enter “Don’t know” in the name field. Next, select “Other” for agency type, and enter zeros for staff hours.

Section C: AFI Project Features

This section asks about the AFI project features. Some of these issues may still be in the design stage, but answer them as to what was written in the grant application or your best guess at this time. If the answer is unknown, then select “Don’t know” where it is offered as a response, or where available use the text box next to the “Please explain variation” response to explain the situation.

For this section:

  • Answer all questions.
  • Asset-specific training. If C.23, C.27, C.30 and C.33 are required but unknown, then enter 999.
  • Enter a zero for all follow up questions (i.e., any question that asks for “Number of Participants”.).

Section D: Characteristics of AFI IDA Account Holders

This section asks about account holder characteristics. If there are no participants, follow the instructions below to reach completion status for this section.

For this section:

  • Select a “Yes” response for D.1.
  • For D.2 – D.5, if no activity or participants, enter zeros.
  • Respond “No” to “Did grantee collect data on this topic?” for the following questions:
    • D.6, D.10, D.18, D.25, D.33, D.41, D.53, D.58, D.66, D.75, D.81, D.84, D.87, D.90, D.93, D.96, and D.103.
  • Enter a zero for any question that asks you to indicate the number of AFI IDA Account Holders.  

Section E: Project Reserve Fund

This section asks about the AFI Grants, Non-federal funds and the bank account(s) that make up your Project Reserve Fund. 

For this section:

  • Complete E.1 – E.2.
  • E.3 – E.4: If the Project Reserve Account is not yet set-up, enter zeros.
  • Click on the “Add Financial Institution” button. You are required to enter information for at least one bank account. If the Project Reserve Fund is not yet set up, enter the name of the bank that you expect will hold the Project Reserve Account. If the financial institution which holds the Project Reserve Account is unknown, enter “Don’t know” in the box for the name of the financial institution. Enter a zero for amount on deposit.

Section F: AFI IDA Account Holder Savings

This section asks about account holder savings. If there are no participants, follow the instructions below to reach completion status for this section.

For this section:

  • Complete F.1 – F.2 (Note: F.3 will pre-populate based on the responses to F.1 and F.2).
  • If there is no difference between F.1 and F.2, then respond “No” to F.4 –F.8.
  • Enter zeros for F.9 – F.12.
  • F.13: Enter “No” to “Did grantee collect data on this topic?” and leave F.14 blank.
  • Click on the “Add IDA Financial Institution” button. Enter the name(s) of the financial partners, or the financial partners you expect to use, and then zeros for the number of open accounts and current deposits. If the financial partners are unknown, type “Don’t know” in the box for the name of the financial partner, and enter zeros for the number of open accounts and current deposits.

Section G: AFI IDA Account Holder Withdrawals

This section asks about account holder withdrawals. If there are no participants, follow the instructions below to reach completion status for this section.

For this section:

  • G.1, G.7, G.13 and G.19: Select “Yes” for all asset purchases allowed for your project. These responses should be the same as the responses to items C.10-C.13.
  • For any section with a “Yes” response (i.e., G.1, G.7, G.13 and G.19), enter zeros for all the follow-up questions.
  • G.25 – G.26: Enter zeros.

Section H: Financial Data Summary Sheet

  • Select “Yes” for each portion of section H and save.

Section I: Characteristics of Account Holders Making a Matched Withdrawal in the Past Year.

Do not complete this section.
